A contact form lets website visitors send you an inquiry. You build each form on the
[**Contact Forms**](/go/contact-form) page in the sidebar.

## Create a form

### Start from scratch

Use this path to choose each field yourself.

1. Click **New form**.
2. Enter a **Name** for the form.
3. Optionally, enter a **Description**.
4. Click **Create form**.

The new form appears first in the list, published, with no fields. Lead creation starts
off; turn it on under **Lead Capture** for leads.

### Start from the Quick start template

Use the template for the common fields.

1. Click **Quick start**.
2. Enter a **Form name**.
3. Turn off **Collect phone number** or **Collect address** to skip that field.
4. Turn on **Make phone required** or **Make address required** if the answer is mandatory.
5. Click **Create form**.

The form is published with First Name, Last Name, Email, and Message fields, plus any
Phone or Address field you kept. The template also turns on lead creation. See
[**Introduction to contacts**](/help/contacts/introduction-to-contacts) and
[**Introduction to leads**](/help/leads/introduction-to-leads).

## Add and order fields

### Add a field

Add fields from the form card.

1. Click **Add field** beside the Fields heading.
2. Enter the **Label** visitors will see.
3. Pick a **Type** in the "Choose a Field Type" window.
4. Fill in **Placeholder** or **Options** (separated by commas), if offered.
5. Choose **Saves to** to copy the answer to a contact or lead.
6. Turn on **Required**, and **Half width** if offered.
7. Click **Add field** to save.

The new field goes to the end of the list.

### Change the order

Drag fields to reorder them.

1. Drag a field by its grip icon to its new place.
2. Repeat for any other field you want to move.
3. Click **Save order**, or **Cancel** to discard the change.

Once **Save order** succeeds, the buttons disappear.

## Edit or delete a field

Edit a field from its row.

1. Click the pencil icon on the field.
2. Change the settings you need.
3. Click **Save**.

The row returns to its normal view with your changes.

To delete a field, click its trash icon. There is no confirmation step.

> **Note:** The **Delete** button in the Fields heading row deletes the whole form
> after you confirm.

## Set the form's settings

Change the settings you need, then save them.

1. Find the section you need: **Details**, **Appearance**, **Lead Capture**, or **Consent**.
2. Change the settings you want.
3. Click **Save settings**, or click **Reset** to discard your changes.

The save bar disappears once the save succeeds.

- **Details** sets the **Name**, the **Description**, and the **Submit button text**.
Leave it blank to keep the default label.
- **Appearance** sets **Accent color**, **Theme**, **Corners**, **Field style**,
**Button style**, **Density**, **Header alignment**, and **Font**.
- **Lead Capture** holds the **Create a contact and lead from each submission** toggle. Once
it is saved on, choose a **Target pipeline** and, if that pipeline has stages,
  a **Target stage**.
- **Consent** adds an optional opt-in checkbox for text messages. Visitors can leave it
unticked, and it never blocks a submission.

> **Note:** Lead creation needs fields that save to "Contact first name",
> "Contact last name", and "Contact email". Without all three, a submission saves but creates
> no contact or lead.

## Publish and embed the form

Publish the form, then copy its embed code onto your website.

1. Turn on the publish toggle in the card header, beside the badge.
2. Click **Copy** under **Embed**.
3. Paste the snippet into your website's page.

The badge reads **Published**, and the snippet is copied.

Visitors see only published forms. The embed is fully responsive and does not inherit
styles from your site's pages.

If your site sends a Content-Security-Policy header, allow the JobWise address shown in the
**Embed** section to frame the form. For placement steps, see
[**Add a form to your website**](/help/contact-forms/add-a-form-to-your-website).

## Where submissions go

Each submission is saved with its answers and any uploaded files. Open **Responses**, beside
the Fields heading. See
[**View form responses**](/help/contact-forms/view-form-responses).

With lead creation on, each submission creates a contact and a lead in the pipeline and stage
you chose. Without a usable pipeline and stage, the contact is kept with no lead.

Each submission also notifies your team by email and push. Every member gets it by default,
and can turn either channel off under **Leads and Messages** in notification settings.

## Field and access limits

**Contact Forms** appears in the sidebar only if your role can see **Lead Pipeline**.
Creating, editing, reordering, publishing, and saving settings need edit access. Deleting a
field or form needs full access.

**Saves to** options depend on the field type:

- Checkbox, Checkboxes, Date, Date & Time, and File Upload: only the "Nothing" option, which keeps the answer on the submission.
- Address: only "Contact address".
- Email Address: "Contact email", "Contact first name", "Contact last name",
  or "Contact company".
- Phone Number: "Contact phone", "Contact first name", "Contact last name",
  or "Contact company".
- Short Text and Long Text: "Contact first name", "Contact last name", "Contact company",
  "Contact notes", or "Lead notes".
- Number and Dropdown: "Contact first name", "Contact last name", or "Contact company".

A File Upload field accepts up to 5 files, each up to 10 MB, and 25 MB per submission.
Images can be JPEG, PNG, GIF, HEIC, HEIF, or WEBP. SVG files are never accepted.
**Allowed files** defaults to "Images and documents". You can choose "Documents" or
"Images" instead.
